Welcome to the Application Form for our People Affected By Cancer Advisory Group and Mailing List. Thank you for your interest in sharing our experiences with the South Yorkshire and Bassetlaw (SYB) Cancer Alliance. If you have not already read the supporting information in the Application Pack please do so here before filling in this form: LINK

First we would like to know which group you want apply to out of the following:

People Affected By Cancer Advisory Board: Higher commitment - Advisory Board members meet every 2 months to support the SYB Cancer Alliance to make decisions and advise about the impact on patients and carers of issues being considered by the Cancer Alliance.

People Affected By Cancer Membership List: Lower commitment - receive involvement opportunities via email, including surveys and invitations to focus groups related to Cancer Alliance projects or pathway improvement work

Advisory Board members will receive the same invitations for involvement as the Mailing List so those applying for the Advisory Board do not need to submit 2 applications if they would also like to be added to the Mailing List.
